Antenatal Diagnosis and Management of Fetal Mid-gut Volvulus

چکیده
Fetal volvulus is an uncommon cause of bowel obstruction, which is rarely detected by an antenatal ultrasound scan. We report a case which presented with fetal stomach and small bowels dilatation at 33 weeks of gestation detected by ultrasonography. She was referred to our unit for safe delivery at 34 week of gestation following a spontaneous rupture of membranes and signs of fetal distress. Delivery was accomplished by Cesarean section. Postnatally, the infant was surgically explored with resection of ileum and end to end anastomosis because of bowel gangrene and volvulus. Ultrasound diagnosis during pregnancy with fetal bowels dilatation is an important tool and may lead to early diagnosis and optimal management of intestinal obstruction.

Intestinal dilatation in the fetus.
Correspondence to: Mr SJ K Holmes. Introduction There are now sufficient data on the sonographic appearance ofnormal fetal gut to allow intestinal abnormality in the antenatal period to be diagnosed, although as yet relatively little attention has been paid to the implications for the child. We present eight such cases. Six children were found to have midgut atresia, three of whom had evidence ...
متن کاملSmall bowel volvulus in mid and late pregnancy: can early diagnosis be established to avoid catastrophic outcomes?
Volvulus in pregnancy is rare and difficult to diagnose. Delayed diagnosis would result in high maternal and fetal mortality. Here we present an unusual case of small bowel volvulus in late pregnancy timely managed by emergency Cesarean section and derotation with excellent maternal and fetal outcomes. Volvulus should be considered in patients complaining ongoing abdominal pain, nausea, vomitin...
متن کاملSigmoid colon volvulus in pregnancy: a case report
Background: Sigmoid volvulus is a rare complication in pregnancy. The clinical presentation is similar to that of non-pregnant volvulus, although the symptoms can be masked by the enlarged uterus and physiological changes during pregnancy and it can be challenged and delayed in diagnosis and treatment. Delay in diagnosis can also lead to ischemia, necrosis and perforation of the intestine, whic...
متن کاملIntestinal malrotations: a review and report of thirty cases.
Intestinal malrotation is a developmental anomaly affecting the position and peritoneal attachments of the small and large bowels during organogenesis in foetal life. It has been defined as absent or incomplete rotation and fixation of the embryonic gut around the superior mesenteric artery. In the present paper, we review the definition, history, embryology/aetiology, epidemiology, symptoms an...
متن کاملAntenatal diagnosis and management of foetal intestinal volvulus.
In-utero intestinal volvulus is a rare but potential life threatening foetal complications. It is a surgical emergency and delay in diagnosis or treatment can increase the morbidity and mortality to the foetus. We report a case of mild foetal bowel dilatation diagnosed at 21 weeks of gestation. She was closely follow up and at 31 weeks of gestation, in-utero intestinal volvulus was diagnosed wi...
